Scabby lesions on the lips of a kid.

Histopathology demonstrated intense epidermal hyperplasia associated with intense parakeratotic hyperkeratosis along with thick superficial degenerated keratin crusts mixed with an inflammatory exudate. Subcorneal vesicle formation was observed forming confluent subcorneal pustules. In the tongue a more incipient lesion was observed consisting of hyperplasia and hyperkeratosis of the epidermis with intraepithelial pustules. The lamina propria had an lymphoplasmacytic and histiocytic inflammatory infiltrate.

These lesions are characteristic of infectious pustular dermatitis or ecthyma contagiosum (Orf) caused by Parapoxvirus. It is a zoonotic disease.

Crusty lesions on the upper lip and whitish lesions on the tongue, the disease is also known as Scabby mouth.

Photomicrograph of an hematoxylin and eosin stain. Labial hyperplastic epidermis and hyperkeratosis. Subcorneal pustules are observed (arrow) and thick surface crusts (asterisk).

A new case of ecthyma in a kid. You can see the characteristic lesions both at the level of the lips and gums.
