Scabby lesions on the lips of a kid.
Histopathology demonstrated intense epidermal hyperplasia associated with intense parakeratotic hyperkeratosis along with thick superficial degenerated keratin crusts mixed with an inflammatory exudate. Subcorneal vesicle formation was observed forming confluent subcorneal pustules. In the tongue a more incipient lesion was observed consisting of hyperplasia and hyperkeratosis of the epidermis with intraepithelial pustules. The lamina propria had an lymphoplasmacytic and histiocytic inflammatory infiltrate.
These lesions are characteristic of infectious pustular dermatitis or ecthyma contagiosum (Orf) caused by Parapoxvirus. It is a zoonotic disease.
